How have you been over the last two weeks?

These short questionnaires (the PHQ-9 for depression and the GAD-7 for anxiety) are used across NHS talking therapy services. They can help you put into words how you’ve been feeling.

Before you start

  • It takes about 3 minutes. Answer based on the last two weeks.
  • Your answers are scored on your device and aren’t sent anywhere. You can choose to save your scores afterwards.
  • This isn’t a diagnosis. It’s a starting point for understanding how you feel, and for a conversation with a GP or therapist.

About these questionnaires

The PHQ-9 and GAD-7 were developed by Drs Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues, with an educational grant from Pfizer Inc. No permission is required to reproduce, translate, display or distribute them.

Scores are grouped into bands (for example “mild” or “moderate”) using the standard published cut-offs. Only a qualified professional can make a diagnosis.
